Palliative Care

Libertana specializes in providing outstanding pediatric palliative care services for patients living at home with complex medical conditions including chronic illnesses, life-limiting conditions, and/or life-threatening conditions. This program works toward relieving symptoms, minimizing hospitalization, and enhancing overall quality of life for the child and family.  

We accomplish this by offering comprehensive support services to both the patient and the family members including:

  • Case management
  • Advocacy and referrals
  • Music therapy
  • Art therapy
  • Massage therapy
  • Nursing
  • Counseling

Rest assured, our holistic and compassionate approach supports the physical, social, emotional and spiritual well-being of patients and families coping with medical conditions. 

This service is fully covered by Medi-Cal and Medi-Cal Managed Care Plans; however, types of services authorized may vary depending on the patient’s insurance plan. Patients who currently do not have Medi-Cal may qualify for services. Please speak with our team for more information.

Palliative Care: What is it?


Palliative care can be utilized for both children and adults who are faced with a chronic or life-threatening illness. It’s designed to enhance comfort and improve the overall quality of life of patients and their families struggling with a serious illness.

In addition, palliative care alleviates the physical and psychological pain experienced in response to serious illness. This care is provided by a qualified team of doctors, nurses, social workers, chaplains and therapists who work together with the patient’s doctors to provide an extra layer of support at home.

This program provides support alongside the patient’s primary medical team, California Children’s Services, Medi-Cal Waiver Programs, EPSDT, and Regional Center Services.

The Difference Between Palliative Care and Hospice Care

Both palliative care and hospice care provide comfort and compassionate care. Hospice care is reserved for terminally ill patients when treatment is no longer an option, usually during the last six months of life. Palliative care can be provided for non-terminal patients while still continuing to receive active treatment for their condition. The goal of palliative care is to help patients live longer, happier lives.
