Case Management

Each individual is assigned a Registered Nurse Case Manager.  The RN Case Manager begins the treatment process by first completing an evaluation based on the physical, emotional, and social needs of the patient.  The RN Case Manager will schedule the assessment with both the buying and receiving customer.  The evaluation includes:

  • A safety review of the home.

  • A complete physical assessment including current medications.

  • Verification of medications, treatments, and physician orders.

After the evaluation is completed, the RN Case Manager will then work with the client’s physician to devise a treatment plan best suited for that individual.  After this plan is carefully completed, the Registered Nurse Case Manager will contact the patient to review the results of the evaluation, discuss the individualized service plan, and work with the patient and family members to schedule services.

Our RN Case Managers are very sensitive to the fact that our clients are making significant changes to their lifestyles.  Therefore, our RN’s devote considerable efforts to minimize the required adjustments.

The RN Case Manager remains a constant asset to the home care team.  By supervising and/or participating in all future care, the RN Case Manager continues to be a companion and stable force in the home care process.
